Provider Application identifying the legal entity, locations and regulated activities
Registered Manager Application where required by the provider’s circumstances
Statement of Purpose describing the service, people and activities
Applicable insurance evidence and core policies: complaints, consent, equality, governance, infection control, medicines, recruitment and safeguarding
Financial viability statement unless exempt; building control evidence where applicable

Requirements for this service

X-ray, MRI and ultrasound examinations fall within diagnostic scope, including keepsake pregnancy ultrasound.
Independent reporting providers may require their own registration.
Mobile and fixed facilities need the appropriate location arrangements.

Registration depends on your activities, legal entity and service model. Confirm the applicable submission checklist before applying. Keep additional assessment evidence ready; do not send documents CQC has not requested.
